Bring to Appointment

• Your child's current immunization record
• All of your child's past medical records
• Your child's current insurance information
• Parent/guardian's drivers license
• New patient registration forms
• Please note a parent or legal guardian MUST be present for your child's first visit to Steeplechase Pediatric Center. We will provide an authorization for non-parent consent to treatment form if you would like a relative to bring your child for their subsequent appointments.
• Ensure the doctor listed as your child's PCP (for HMO, CHIP or Medicaid) is a Steeplechase Pediatric Center doctor.

Please do your best do arrive 15 minutes earlier than your appointment time, especially if you need to update your demographic information.

Please note the following fees:

• All visits without appointment will incur $25.00 fee.
• If you have an appointment and cannot make it, please contact us to either cancel or re-schedule your appointment. Please note we require 24 hours notice prior to cancellation. There will be a $25.00 charge if you fail to do so.
• All appointments after regular business hours, weekends and all Federal Holidays are subject to $20.00 fee.
• All calls to the answering services that are sent to on-call physician are subject to $15.00 fee.
• Transfer of medical records from Steeplechase Pediatrics to any other medical practice, physician or hospital is a subject to $27.00 fee.
