If you are a new patient, please call one of our convenient locations to make an appointment. To facilitate the process, you may choose to print and complete all of the necessary forms prior to your visit.

On-Line Access/Patient Portal
Our Patient Portal now offers secure and convenient way to pay your medical bills online! With secure online access you can also experience the convenience of:
- Requesting a refill
- Reviewing and print immunization records
- Reviewing medical history and normal lab results (we will call you for abnormal results)
- Updating demographic information
To register, just ask any of our friendly staff for assistance. You will be supplied a username and password, then you are on your way to explore what our Patient Portal has to offer!
Please fill in all fields on the online appointment request, including providing a copy of your most up-to-date insurance card. Without these completed, your appointment may not be scheduled.
